HowTo Create a contact form

Nov 172007

A contact form is a must have on all websites. You can realize such a form with build in functionality, even if you have anonymous "logins" on your SharePoint site.

Steps to create a contact form:

  • Create a custom list, and configure it with all the fields you need.
  • Change the security settings in the advanced settings for the new contact list

Now everything is set up for your contact form. But how do you get knowledge of a new item in your list? You can not configure an alert, because you are only allowed to view your own posts. But you can add the RSS feed for the list into your Outlook to be notified if someone tries to contact you.

Hi there !

First of all thanks for your post, it´s a really useful tip. I´m trying to create the same scenario for a "send your CV" form; I have created a custom list with the required fields, and changed the security settings in the advanced settings for that list, as you learly explained in your post.

All in all, when I try to access the URL of NewForm.aspx (with the ?Source=.. included) as an anonymous user, it keeps asking me for credentials.

I don´t know what the problem is, maybe a need to configure something special in the global annonymous access to the web?

Thanks in advance for your help.

Hi,
in my Site Collection I set anonymous access to "entire site" (the first option). Other than that, I did not change settings.

Hi there,

The post was really helpful, thanks for posting it. How can we assign a custom master page/theme to the contact us page? I have created a custom master page for the "pages" in my site, but i am unable to apply it to the list "contact us" page above(it takes the default system master page and theme). Any suggestions would be greatly appreciated.

Kind regards,
judef

http://www.heelys.com/Pages/Search.aspx?k=where or http://www.culturaonline.pt/Pages/pesquisa.aspx?k=proto

Any help would be greatly appreciated.

The list settings above will not work for anonymous users. The Anonymous Access permission levels will be greyed out.

I believe you will need to set Read Access to All Items.

What I'd really like to see is an Email feedback form with CAPTCHA and validation. Similar to what you have here. :)

Great, simple stuff. Much thanks.

I've been experimenting, and created a "Name" column with type "Person or Group" (the People Picker). It works great, reading AD and passing the selected name to the list upon submission.

Now, however, I'm getting greedy. I want to add an "Email" column, but I want it to automatically fill in when the "Name" is selected, based on the "Name" user's AD profile.

Any recommendations?

Again, much thanks!
